Due to NYS Law & Golden Tans company policy...
The following will take effect as of 10/1/06...

• All clients will be required to sign a consent form every 12 months.

• All clients between the ages of 14 and 18 will be required to have a consent form signed by a parent or guardian, in the presence of a Golden Tans employee, every 12 months.
  (Parent or guardian must be present. Notes from parent or guardian will not be accepted)

• All clients will be required to show a valid photo ID, verifying their Date of Birth, prior to each visit.

• All clients will be required to show FDA approved protective eyewear prior to each visit.

• No one under the age of 14 may tan in tanning beds and booths.

• Clients under the age of 14 may tan sunless in our Mystic Tans®, California Tan®, and Vera Spa® spray-on booths and Custom Airbrushing. (Parent or guardian must be present.)
